Southwest Ohio Health Partners is an innovative health care facility offering an integrative treatment approach to treating all types of body pain and musculoskeletal dysfunction. 

We specialize in sports medicine, interventional pain management, regenerative medicine (including treating most forms of pain – headache, neck, back, pelvic, muscle/joint and other pain) and musculoskeletal dysfunction. Our trained staff utilizes conventional and alternative medical techniques to create a health plan created specifically for you. Meet Dr. Tricia Croake-Uleman, M.D.

Southwest Ohio Health Partners is led by medical director Tricia Croake-Uleman, M.D., who practiced medicine since 1998 and has extensive experience in interventional pain management, sports medicine, regenerative medicine and family medicine. Before opening this clinic in 2008 she spent four years performing thousands of non-narcotic injections in Phoenix, Arizona. Earlier in her medical practice, Dr. Croake-Uleman provided family medicine services including sports medicine, pain management, health maintenance, gynecology, pediatric, adult and geriatric care and urgent care.

Dr. Croake-Uleman’s education includes her earning a B.S.E. in Biomedical Engineering at Arizona State University in Tempe, Arizona; an M.D. at Wright State University School of Medicine in Dayton, Ohio; and completing her residency at Bethesda Family Practice in Cincinnati, Ohio. She further trained in interventional pain management, sports medicine, regenerative medicine including platelet rich plasma (PRP), prolotherapy, stem cells, Lyftogt perineural therapy (neural prolotherapy), neural therapy and acupuncture.

Dr. Croake-Uleman is a board certified physician. She has been a Diplomat of the American Academy of Pain Management since 2007, American Board of Family Medicine since 2001 and Diplomat of the International Academy of Medical Acupuncture since 2009. She is a member of the American Society of Interventional Pain Physicians, the American Academy of Pain Management and the American Academy of Orthopedic Medicine. Dr. Croake-Uleman is a certified yoga instructor (RYT200) and is an avid triathlete.

Dr. Croake-Uleman takes medical mission trips to Honduras and Mexico at least annually to help patients with pain and orthopedic issues and to teach other physicians.
